Diary Cards
Complete Your Diary Card
- Click "Diary Card" from the left-hand navigation menu.
- Click "Fill Diary Card".
- Select the Emotions you want to rate the intensity for within a given day.
- Select the intensity of urges you had in a given day and number of times you acted on these urges.
- Select the skills you practiced on a given day.
- Input any relevant information in the free text fields on the final screen.
- Preview your completed Diary Card entry and press "Complete" to submit it.
- Ask your clinician if you have any questions about how to complete the Diary Card.
- Note: Your clinician may not see any information you enter on the Card until they review the Card with you in session.
View Your Diary Card
- Go to the "Diary Card" tab in the left-hand navigation menu.
- Click "History" to access your completed Diary Cards, organized by week. To view a specific Card, click or tap "Preview Weekly View."
Skills Library
View Assigned Skills
- To view skills that have been assigned to you, go to the "My Skills" tab in the left-hand navigation menu. You can view your Individual Skills assigned by your clinician, and your Group Skills assigned by your group leader(s).
My Crisis Plan
Upload Your Crisis Plan
- Go to the "My Crisis Plan" tab in the left-hand navigation menu. This will take you to the Crisis Plan Overview, where you can download a Crisis Plan Template and fill it out on your device.
- Click or tap "Upload PDF."
- Select your completed PDF from your device.
- Click or tap "Upload" and your filled-in PDF will appear on the "My Crisis Plan" tab.
Access Your Crisis Plan
- Go to the "My Crisis Plan" tab in the left-hand navigation menu. This will take you to the Crisis Plan Overview.
- If you've already uploaded a completed Crisis Form, it will be on the first page, "My Crisis Plan." You can re-download the blank fillable form and upload edited versions as often as needed. Your clinician will immediately have access to the most recently uploaded version of your crisis plan.
